Are 55 years old or older.

Require the nursing facility level of care, as defined by the State of California.

Are able to live in the community without jeopardizing the health and safety of yourself and others at the time of enrollment.

There are no deductibles or copayments for any service, medication, or care approved by your care team.

Medi-Cal Only

• No monthly premium.

Medicare Only

• A monthly premium applies, determined by your care team.

Enrollment Process

1. Talk to one of our representatives about enrollment

   Call us at 1-855-586-7336 (TTY: 711). Visit us at 1839 W Valley Blvd, Alhambra, CA 91803. We can provide transportation for you to come to our center.

2. Review eligibility with our intake team

   We will assess your healthcare needs and complete an evaluation to determine your eligibility. We will submit documents to the state for approval.

3. Enroll in Seen Health

   Once approved by the state, you will sign the enrollment agreement and review your personalized care plan. We will enroll you in Seen Health! Your enrollment is effective the first day of the following month after you sign the Enrollment Agreement. For example, if you sign the Enrollment Agreement on March 14th, your enrollment will be effective on April 1st.

Disenrollment

You have the right to disenroll from Seen Health without cause at any time.

We may also terminate your enrollment with Seen Health for various reasons.
